AUTHORIZATION and ACKNOWLEDGEMENT

The undersigned hereby certifies that the information provided above is true and complete. Mckinney Trailers may receive from and disclose to other persons, including credit reporting agencies about the above named applicant’s account and credit experience, and the undersigned authorizes any person to release to Mckinney Trailers credit information on the above-named applicant upon Mckinney’s request.

In consideration of being allowed the privilege of an open credit account, the undersigned, an authorized representative for the above named company applying for credit, hereby agrees and accepts the Standard Terms and Conditions of Mckinney Trailers, including but not limited to the payment terms listed in the Standard Terms and Conditions. Mckinney’s Standard Terms and Conditions will apply to all transactions with Mckinney involving the lease or rental of Mckinney Equipment, whether pursuant to a Long-Term Lease Agreement, National Account Agreement, Short-term Rental Agreement, Equipment Rental Agreement or any other agreement.

This agreement and electronic acceptance shall be governed by the laws of the State of California.
